How to do Uttar Pradesh Nursing Council registration

U.P. Nurses & Midwives Council has a beautiful website that gives easy access for the candidates from anywhere ( click on LInk). To register with the nursing council after the completion of your course, an application form can be downloaded from the nursing website.

Guidelines & Instructions for Uttar Pradesh Nursing Council Registration

1. Use Black/Ball pen for filling up the form.
2. Stick Colored passport size photograph, do not staple or pin.
3. Fill the form in English capital letter & number in English numeric character.
4. Do not use abbreviation 
5. Since it is a ICR form, please do not fold.
6. Photograph & Signature should be attested by Principal of Training Center.

Nurses of the state (U.P) (Only For B. Sc Nurses )

• Age proof certificate (photocopy of high School certificates and inter mark sheets.
• Photo copy of all year mark sheets.
• Provisional Certificate from university.
• Course completion Certificate from the institution.
• Affix photograph & signature in the box mentioned in the form and get it duly attested by PRINCIPAL of the College.

Nurses of the other state (G.N.M./ B. Sc / A.N.M/ H. W.(F) Nurses

• Photo copy of Registration certificates from parent council.
• Photo copy of Employer I.D. Proof from Institution.
• Age proof certificate (photocopy of high School certificates and inter mark sheets.
• Photo copy of all year mark sheets.
• Provisional Certificate from university.
• Affix photograph & signature in the box mentioned in the form and get it duly attested by PRINCIPAL of the College.

For duplicate Registration from Uttar Pradesh Nursing Council

• Copy of F.I.R.
• Publication in news paper.
• Affidavit.
• Signature & photograph should be attested by- principal /attesting authority.

For registration of additional Qualification (Only for Post Basic B.Sc Nursing / M.Sc. Nursing / Post Basic Diploma Courses)

• Photo copy of U.G. Degree Registration Certificate of U.P. Nurses & midwives council.
• Photo copy P.G. Degree Provisional Certificate from the university/Board.
• Course completion Certificate from the institution.
• Photo copy of all year mark sheets.
• Affix photograph & signature in the box mentioned in the form and get it duly attested by principal of the College.
