Procedure to get Duplicate Kerala Nursing Registration Certificate
In case you lost your Kerala Nursing Registration Certificate , and want to get to a duplicate certificate of the same, you need to follow the following
1. First you have to submit a application to registrar stating that you lost your original certificate, the circumstances led to it. Please also include your telephone number in the application
2. You have to produce the original newspaper ad, that you published in newspaper stating that you lost your certificate
3. The original copy of Police case record
4. An affidavit on 100 RS. document paper from a First Class Magistrate or Nottery
5. The copy of lost certificate
6. A DD worth Rs. 2000 drawn in favour of Registrar Kerala Nursing Council - SBT Trivandrum Central Branch.
7. The applicant must be present in front of the Nursing Council when asked to do so.
