Procedure to get a duplicate registration or mark list from Punjab Nurses Registration Council

I would like to share a short note on "how to get a duplicate certificate from Punjab Nurses Registration Council or PNRC". It can be a Detailed Mark list, Registration Certificate, Diploma /Degree Certificate or Subject Certificate. The process and application is same for all the certificates.

Please fill in the required field and the attach an attested photograph of the candidate. Application form and  the photo should be attestested by the Principal of the institute where the course is completed. You should have an extra passport size photograph with you, there is no need to attest it.
